Waiver

Safeguarding Our Health We have taken extra precautions, and we’ve implemented new guidelines to ensure the health and safety of all those who choose to use our facilities and for our staff.

Additionally, all members will be required to complete a waiver upon their first visit for each member of your household. You can complete the waiver below and submit electronically prior to your first visit or you can sign the waiver on your first day back. Please note that members without waivers will be required to sign this additional waiver before entrance to the facility. Should you choose to not sign the waiver, we, unfortunately, will have to deny your facility access until the waiver is signed.
